PD-6 DNA methylome as a potential biomarker in biliary brushes and bile fluid samples to differentiate between benign and malignant biliary stenosis

چکیده
The diagnosis of perihilar and distal cholangiocarcinoma (CCA) remains an important clinical challenge. Current detection methods are mostly based on biliary brushing samples with suboptimal sensitivity and/or specificity, leading to late higher mortality rates. We aimed identify a diagnostic biomarker using targeted DNA methylation sequencing in classical training validation study-design. Biliary bile fluid from patients known malignant versus benign stenoses were prospectively collected during endoscopic retrograde cholangiopancreatography (ERCP). Clinical data including baseline patient characteristics follow-up was recorded. All subjected targeted, enzymatic (EM-seq) total 608,293 capture probes targeting genomic regions be hypermethylated cancer. Differential analysis used differentially methylated between samples. Only both brush retained. These train ‘random forest classification’ prediction model ‘training cohort’ samples, 10-fold cross-validation. remaining then as ‘validation set’ test the potential score classifying Receiver operating characteristic curve analyses evaluate performance brush-derived fluid-derived scores differentiating A 43 included November 2019 September 2021. Twenty-eight had stenosis, majority which due ischemic cholangiopathy, while 15 stenosis CCA or pancreatic adenocarcinoma. Average coverage significantly (59.6X 24.9X, respectively; p < 0.001). identified 669 sample types. Brush-derived differentiated specificity 0.913 0.933 (AUC 0.93). Similarly, derived demonstrated 0.961 0.8 0.89). present DNA-methylation that accurately differentiates stenosis. Bile aspiration ERCP is alternative when not feasible. Further larger cohorts warranted.

Biochemical profile of bile fluid in patients with malignant cholestasis in comparison with cholestasis due to biliary stone
Background: Cholangiocarcinoma is an invasive biliary malignancy with poor prognosis. Diagnostic accuracy of conventional methods is low which is mainly due to the specific anatomy of the disease. The aim of this study was to evaluate the diagnostic value of biochemical profile and tumor marker of the bile in patients with malignant cholestasis compared to that of choledocholithiasis. ...
متن کاملDetection of Helicobacter DNA in Bile Samples of Patients with Biliary Diseases Living in South of Iran
Background: It has been reported that several species of Helicobacter colonize the biliary tract of animals and human, but their participation in hepatobiliary diseases are not established. This study is undertaken to determine if Helicobacter genus members, especially with regard to H. pylori, could be detected in the bile and gallbladder diseases. Methods: Thirty-eight gallbladder tissues and...
متن کاملAssociation between Helicobacter bilis in bile and biliary tract malignancies: H. bilis in bile from Japanese and Thai patients with benign and malignant diseases in the biliary tract.
Japan and Thailand have high incidences of bile duct carcinoma and gallstones. The presence of Helicobacter bilis (H. bilis) detected by polymerase chain reaction (PCR) and 16S rRNA analysis in bile samples from Chileans with chronic cholecystitis was reported. The association between H. bilis in bile and biliary tract malignancies has not been investigated, and therefore the aim of this study ...
متن کاملIdentification of biliary bile acids in patients with benign biliary diseases, hepatocellular carcinoma and cholangiocarcinoma.
Bile acids are implicated as aetiological factors in many types of gastrointestinal tract cancer including cholangiocarcinoma (CCA). Alterations in bile acid concentrations may affect the pathogenesis of these different types of cancer. Our aim was to determine the bile acid profile in gallbladder bile from patients who underwent liver resection. Thirty-seven patients with cholangiocarcinoma, 5...
متن کاملRoles of serum and biliary CEA, CA19-9, VEGFR3, and TAC in differentiating between malignant and benign biliary obstructions.
BACKGROUND/AIMS Despite the presence of many diagnostic methods, the differential diagnosis between benign and malignant biliary obstructions is still not easy. We aimed to evaluate the role of serum/biliary carcinoembryonic antigen (CEA), carbohydrate antigen 19-9 (CA 19-9), vascular endothelial growth factor receptor-3(VEGFR-3), and total antioxidant capacity (TAC) tests in this differential ...
